This is a REQUEST FORM. ALL APPOINTMENTS MUST BE CONFIRMED MANUALLY. Appointment will not be booked until you recieve correspondence from artist and it is confirmed.

Booking Policy

A $30 non- refundable deposit is required to secure your appointment at the time of booking. This will go towards the final price of your tattoo. Deposits are non-refundable unless you cancel within 72 hours (3 days) prior to appointment. IF you need to reschedule, you MUST notify me within 72 hours (3 days) in advance to transfer your deposit to a new date ( one time reschedule only). If in any instance you need to reschedule more then one time, another $30 deposit will be required and the initial deposit will be considered forfeited.


No call/ No shows or last minute cancellations (under 72 hours) will result in a forfeited deposit. There is a 15 minute grace period at appointment time. If you are more then 15 minutes past your scheduled appointment time your APPOINTMENT WILL BE CANCELED.
